Skilled Nursing

  • Coordination and implementation of physician's plan of treatment
  • Routine patient evaluation and communication to physician regarding patient's response to care, and changes in patient's condition
  • Interaction with patient's family/caregiver, co-workers and other disciplines to provide observation and feedback reagarding patient's condition
  • Review of medication regimen and patient training


Physical Therapy

  • Assisting physician's with initial and on-going patient evaluation
  • Directing and aiding patients in various exercises and activities
  • Gait training and physical rehabilitation
  • Assess and instruct on safe use of durable medical equipments, prosthetic, and orthotic devices
  • Coordinating physical therapy with other team members

Occupational Therapy

  • Evaluating patient's level of function
  • Guiding the patient in therapeutic, creative and self care activities
  • Helping the patient adapt to age-related changes such as decreased energy and vision
  • Assisting patient to increase physical strength and endurance to maintain self sufficiency
  • Identifying and reinforcing the older adult's abilities for independent living

Speech Therapy

  • Coordination of plan of treatment with the patient's physician
  • Voice disorder treatments, Speech articulation disorders, Dysphasia treatments, Language disorder treatments, Aural rehabilitation, Non-oral communication

Medical Social Worker

  • Coordinating with the home health care team regarding the patient's psychosocial assessment and collaborating with necessary resources to meet the patient's needs
  • Provide patient/caregiver access to community resources to promote independent living
  • Assisting patients and their families/caregivers in managing personal and environmental challenges, which prevent the patient from fully benefiting from home health care

Caregivers

  • Communicating with the clinical team regarding all patient observations to improve care and treatment
  • Assisting patient with ambulation and/or transfer, using proper body mechanics to protect patient and personal safety
  • Assisting patient in meeting personal hygiene needs
